The labial frenulum normally attaches above the non-moving gums. But it often attaches to the center of the upper lip and between the upper two front teeth. This can cause a large gap and gum recession by pulling the gums off the bone. A labial frenectomy removes or relocates the labial frenulum. Orthodontic patients often have this procedure done to assist with closing a front tooth gap.
Besides orthodontic usage, patients with denture can benefit from labial frenectomy. When a denture patient's lips move, the frenulum pulls and loosens the denture which can be uncomfortable. This surgery is often done to help dentures fit better.
Intial Situation before frenectomy. The gap between two incisors can not be closed. |
A 14 years old Asian girl has a wide central diastema between her central incisors. The orthodontic treatment has been started a while ago. The gap can not be closed with orthodontic teeth movements because of the ´high´ attachment of the labial frenulum. The frenulum need to be removed and the attachment repositioned at a ´lower position´, ie. inside the upper lip, so that the gap can be closed.
